Use it on the floor for each task that moves a load. Then file it with your hazard assessment.

Before you start

List each task where someone lifts, carries, pushes, pulls or holds a load.
Ask the crew which tasks leave them sore.
Pick a trained person to lead, and bring in workers who do the task.

For each task

Name the task and where it happens.
Record the load: weight, size and shape.
Record how often it is moved per shift.
Note start height, end height and reach.
Note twisting, bending and carry distance.
Watch several workers and several repetitions, late in the shift too.
Note trunk lean, knee alignment, balance and pace.

Rank and control

Rank the task against the others by likelihood and how often it is done.
Fix the task first: height, lift aid, shorter reach, smaller load.
Then add procedures, training and rotation.
Protective equipment comes last.

Record and follow up

Write down what you assessed, what you found and what you changed.
Date the assessment and keep it available at the site.
Tell the crew what changed.
Check the task again after the fix, and date that check.
Review again when the work changes, or when a worker reports symptoms.
